Complex Sleep Apnea

Sleep apnea can be caused by physical blockage of the airway, which is obstructive sleep apnea, or it can be caused by the brain’s inability to properly control normal breathing, which is known as central sleep apnea. Both obstructive and central sleep apneas can occur for many different reasons, but when the causes for both obstructive and central sleep disease are experienced at the same time in a patient, it is called complex (mixed) sleep disease.

Complex sleep disease is often a wide range of problems, often stemming from a previous, untreated apnea condition. An example would be a patient who suffers from obstructive sleep disease developing chronic heart failure, leading to lower blood flow to the brain, making the brain less able to regulate breathing while asleep. In this case, the patient would then be suffering from both the obstructive sleep apneas in addition to the central sleep apnea caused by the brain’s inability to control the patient’s breathing in a normal manner.

The patient would then be suffering from complex sleep apnea, which is much more disruptive to the patient’s sleep and overall health than one type of apnea alone.

Complex sleep issue’s symptoms are often more severe versions of the symptoms seen in obstructive or central sleep issue, even in mild cases, because of the compounded nature of the apneas experienced. For example, when the patient experiences an apnea due to physical obstruction of the airway, a central sleep issue may also be triggered, and for a moment the patient may not even attempt to breathe through the obstruction. The patient would then either be awakened by the lack of breathing and have his/her sleep pattern disrupted, or the patient would experience a hyperpnea (rapid, shallow breathing) to compensate for the lack of oxygen. This cycle would continue throughout the patient’s sleep, leaving the body taxed when it should have been recuperating.

Treating complex sleep apnea is a tentative and experimental field, as complex sleep apnea has only been recognized as a distinct form of apnea relatively recently. Because of this, methods of treatment are still in experimental development, as treatment of traditional sleep disorder such as CPAP and BiPAP, as well as drugs used to treat central sleep disorder, often have detrimental effects on the patient’s condition, usually worsening it.

A few of the treatments of complex sleep disorder currently in the experimental phase are a PAP Gas Modulator device, which introduces small amounts of CO2 to stabilize CO2 homeostasis in the patient, as well as an ASV device, which rapidly changes the pressure of the air the patient is receiving, evening out breathing and eliminating the erratic ventilation that would otherwise occur. Though both of these methods show promising results, only ASV has been approved by the FDA in the United States, and even then, they are both considered in experimental phase by most physicians.

Complex sleep disorder is a very intricate and complicated sleep disorder. Much care must be taken with the patient to ensure that their condition isn’t further complicated. Although conventional methods of treating sleep disorders are ineffective, science is making headway into treating complex sleep disorder, and new developments as well as current experimental treatments are constantly being advanced.

Mixed Sleep Apnea Symptoms

Most sufferers of sleep apnea experience either obstructive sleep apnea, which is a physical blockage of the airway by tissue in the throat, or central sleep apnea, which is the failure of the brain to control even breathing, resulting in cycles of apneas and hyperpneas. When a patient suffers from both central and obstructive sleep apnea and exhibits symptoms of both, the patient is said to suffer from mixed sleep apnea. Mixed sleep apnea is a tricky form of sleep apnea that is quite difficult to amend with established forms of treatment.

Mixed sleep disease is usually diagnosed when the patient not only has physical blockage of the airway, but also experiences reduced or erratic brain activity in the area that controls breathing. This can cause treatments that are used to reduce one of the normal types of sleep apnea to be ineffective. Treating the symptoms of obstructive sleep apnea, for instance, may still result in the brain being unable to control breathing properly.

The patient, while treated for obstructive sleep disease, is still suffering from central sleep apnea.

Another symptom that regularly shows up in those suffering from mixed sleep disorder is chronic heart failure. Patients who show symptoms of chronic heart failure may experience persistent coughing, a weaker heart rate, and tiredness and shortness of breath, adding to the symptoms of the sleep disorder. While heart failure is also a symptom of severe obstructive sleep disorder as well, it is a symptom that shows up in most cases of mixed sleep disorder.

Mixed sleep disorder can also be induced indirectly by use of opiates. Narcotics such as opiates are often responsible for reduced respiratory activity, therefore leading to cases of central sleep disorder.

Those who use opiates are at a higher risk of suffering from mixed sleep disorder, especially if they already exhibit symptoms of obstructive sleep disorder. Once the brain’s activity lessens and the central sleep disorder begins to affect the patient, the obstructive sleep disorder may worsen and the patient may exhibit even stronger symptoms.

When a patient suffers from one form of sleep issue, the symptoms are usually amplified when the other form becomes present. Increased lethargy, deteriorating heart condition, and restless sleep all become much more pronounced. The patient’s airway is not only obstructed by tissue, but the brain fails to attempt to breathe when the obstruction occurs. When this occurs, increased stress is put on the patient, especially in the heart and brain. The added stress on the heart can cause increased heart failure, which can lead to worsening central sleep issue symptoms. The brain’s sleep cycle is also interrupted when the patient is awakened when the issue is not resolved. Because the brain cannot complete a normal sleep cycle, symptoms of sleep deprivation may begin to appear. Concentration may become harder and learning may become more difficult.

Those who suffer from mixed sleep issue suffer from the symptoms of both central and obstructive sleep issue. This can lead to many health problems when left untreated. Luckily, effective treatment of one form often lessens the symptoms of the other. Once the body is brought back into balance, the patient’s health will often begin to improve, and the symptoms of the apnea become less severe as the patient’s body heals from the damage caused by the mixed sleep issue.

Relaxation

Adverse Conditions For Sleep

How important is a good night’s sleep? Sleep is vital to a person’s mental and physical health.

A study conducted at the University of Iowa showed that sleeping fewer than 7.5 hours a night increased a person’s risk of injury by 61 percent.

Research indicates that teenagers grow while they are actually asleep, their growth hormones are secreted in the deepest phase of slow-wave sleep. They also have different sleep patterns, and sleeping in late is good for them. Could some adolescents could be stunting their growth by not sleeping properly? The American National Sleep Foundation actually presented a symposium on teenagers ‘sleep phase delay’ in June 2005, and the Manatee County School Board, Florida successfully lobbied to delay the start of the school day to cater for his sleep phase delay. Teenagers are at risk of sleeping badly during stressful exam times.

Teenagers who are up early for school, and playing sport on the weekends rarely get the chance to sleep in.

Dr. Mindelll and Dr. Barry Jacobson’s study Sleep Disturbances During Pregnancy showed that by the end of pregnancy 97.3% of women were waking up at night, an average of 3.11 times. And all mothers know that if their baby is not sleeping the whole family suffers. The mother gets tired and cranky, and then feels guilty if she snaps at the other children during the day. A little later comes menopause, hot flashes, and some more sleep challenges.

If you’re a shift-worker, it effects the patients in the hospital, you’re accuracy, and your own long-term health. Recently the Danish government commenced paying compensation to women who have developed breast cancer after years of working night shift. One of the reports published in the journal of the National Cancer Institute showed a 36% greater risk of breast cancer for women who had worked night shifts for more than 30 years, compared with women who had never worked nights. Previously there have been studies indicating higher rates of cardio-vascular disease and gastro-intestinal disadvantages, low birth-weight in babies and longer pregnancies for women. There now appears to be mounting evidence that shift work could suppress the production of melatonin in the body. (Melatonin is believed to have some beneficial effects in preventing the onset of cancer.) If anyone needs optimal melatonin production you would hope it would be shift-workers.

Insurance companies ask whether people suffer from insomnia in their policy documents.

It’s also difficult when there are other medical problems that people are dealing with, at the same time as having disturbed sleep e.g. people in pain from illness, recovering from surgery, living with HIV, tinnitus sufferers, snorers, restless legs syndrome, sleep apnea, hyperthyroidism, Parkinson’s disease, arthritis, asthma, indigestion, coughing, even just a cold. And mental difficulties include dementia, mental health problems, stress, anxieties, bi-polar disorder etc.

Trying to sleep can become more challenged if you are taking a combination of prescriptions – each of which may or may not be totally compatible with the next.

I think the strategy here is to do the best you can with the information you have available at the time. There is no benefit in adding on yet another stress to the situation. Many insomniacs apparently underestimate the length of time that they are actually asleep at night.

So, yes, be totally informed about what you’re dealing with, and what the side effects of your prescriptions are, take whatever measures seem logical and practical, and then do the best you can with your ATTITUDE to the situation you are in. The more positive your attitude, the greater the chance to find some creative results.

Utilize whatever methods are available to make your time more restful and relaxed, by playing great music, relaxing, looking after yourself as best you can, taking appropriate supplements, exercising, eating nutritious food, and being with the people you love.

Unfortunately studies show that most people with this disorder don’t know they have a problem and so this disorder is under diagnosed in the population. Severity varies from person to person, form barely noticeable to obvious.

Medical science cannot diagnose a disease if there are no symptoms and the person does not complain about any difficulties.

Some women seem to have seizures only during pregnancy, but this is rare and, indeed, if you think these women have no part (usually single), seizures in the past, which then become secondary generalized during pregnancy . However, seizures should always be investigated, because there May be the underlying causes.

Long-term insomnia – this can be an effect of environmental chaos like living in a noisy street or caused by the night working shifts. Insomnia

Webster’s dictionary describes insomnia as habitual sleeplessness, inability to sleep. I describe it as miserable. There are few things I can think of that make me more irritable than getting up in the morning feeling like I haven’t even closed my eyes. Sometimes insomnia is difficulty falling asleep, other times it’s the inability to stay asleep. I have suffered from both problems. Because it is a symptom, it means there is an underlying reason, some kind of disorder that is causing the problem.

There are different types of insomnia:

Transient insomnia: Lasts from days to weeks. It can be caused by changes in the sleep environment, timing of sleep, depression or stress.

Acute insomnia: This is the inability to sleep well for a long period of time. Anywhere from three weeks to six months.

Chronic insomnia: Can last for years.

It can be caused by another disorder. Its effects vary, according to its causes. Symptoms might include sleepiness, muscular fatigue, hallucinations and/or mental fatigues.

Some of the patterns of insomnia:

Onset insomnia: Difficulty falling asleep at the beginning of the night.

Middle-of-the-Night Insomnia: Characterized by difficulty returning to sleep after awakening in the middle of the night or waking too early in the morning.

Terminal insomnia: Early morning waking. Often a characteristic of clinical depression.

Probable causes of insomnia:

Drugs or stimulants.

Hormone shifts.

Lifes problems.

Mental disorders.

Disturbances of the circadian rhythm such as jet lag or shift work.

Medical conditions.

Parasomina includes sleepwalking, nightmares.

Insomnia sufferers, their sleep duration and mortality:

A survey done in America found those who reported sleeping 7 hours per night had the lowest mortality rates. Those who slept for fewer than 6 hours or more than 8 had the highest mortality rate. The survey also showed mild to moderate insomnia for most people may actually increase longevity. It is unclear why sleeping longer than 7 hours is associated with excess mortality. According to an item I saw on T.V. researchers have found that some people have a gene that allows them to be fully rested with just 6 hours of sleep. They didn’t give any additional details.

Treatment for insomnia:

As stated earlier, insomnia is usually a symptom caused by another disease or disorder. It is important to identify or rule out medical and psychological problems before deciding on the treatment.

If you suffer from insomnia, ask your doctor about melatonin. It is available without prescription in the United States at your local health food store.

Because it is sold as a dietary supplement, the Food and Drug Administration (FDA) regulations that apply to medicine are not applicable to melatonin.

There are unwanted side effects in some people. This is only one of the reasons it is a good idea to inform your doctor that you are interested in using melatonin for your insomnia.
