Insomnia Webster’s dictionary describes insomnia as habitual sleeplessness, inability to sleep. I describe it as miserable. There are few things I can think of that make me more irritable than getting up in the morning feeling like I haven’t even closed my eyes. Sometimes insomnia is difficulty falling asleep, other times it’s the inability to stay asleep. I have suffered from both problems. Because it is a symptom, it means there is an underlying reason, some kind of disorder that is causing the problem. There are different types of insomnia: Transient insomnia: Lasts from days to weeks. It can be caused by changes in the sleep environment, timing of sleep, depression or stress. Acute insomnia: This is the inability to sleep well for a long period of time. Anywhere from three weeks to six months. Chronic insomnia: Can last for years. It can be caused by another disorder. Its effects vary, according to its causes. Symptoms might include sleepiness, muscular fatigue, hallucinations and/or mental fatigues. Some of the patterns of insomnia: Onset insomnia: Difficulty falling asleep at the beginning of the night. Middle-of-the-Night Insomnia: Characterized by difficulty returning to sleep after awakening in the middle of the night or waking too early in the morning. Terminal insomnia: Early morning waking. Often a characteristic of clinical depression. Probable causes of insomnia: Drugs or stimulants. Hormone shifts. Lifes problems. Mental disorders. Throughout our lives, at various times, almost everyone will occasionally deal with a lack of sleep. Almost always, these periods of sleeping disorder are easily explained by influences like temporary stress, illness, excitement, or other temporary reasons. Which one you use is not nearly as important as being accurate in your record keeping. This record should be kept for a minimum of two weeks to establish your personal patterns and habits. One pattern you should look at is the relationship between the number of hours spent sleeping, how often you woke up during the night, and how tired you felt the next day. Looking at these relationships, you will get a much better idea of just how much uninterrupted sleep you personally need to avoid being sleepy the next day. If you have recorded everything accurately the diary will also help you see other practices and patterns of behavior that might be preventing an uninterrupted, restful nights sleep. If your diary indicates patterns such as you always take more than a half an hour to fall asleep, or you wake up several times each night for long periods then you may have a sleep disorder. Other indications include a practice of taking frequent naps during the day or feeling so sleepy during the day that you fall asleep inadvertently at inappropriate times. A sleep disorder is characterized as the disturbance in time, quality, amount, or behavior associated with sleep. The average length of time an individual sleeps varies but on average most adults sleep between seven and nine hours per night. Not only is the quantity of sleep important but the quality of sleep is also important. People need to sleep in order think clearly and react quickly to everyday situations. There are five stages of normal human sleep cycle. Stage 1 occurs when an individual is falling asleep and is considered a NREM (non-rapid eye movement sleep). About 5 percent of Stage 1 is represented in a normal adult sleep time. Stage 2 marks the beginning of true sleep. About 50 percent of Stage 2 is represented in a normal adult sleep time. Stage 3 and Stage 4 is the deepest level of sleep. About 10-20 percent of Stage 3 and 4 is represented in a normal adult sleep time. Stage 5 is the REM (rapid eye movement) sleep. About 20-25 percent of Stage 5 is presented in a normal adult sleep time. The following is a list of the different types of sleep issues that exist: Insomnia – An individual has difficulty falling asleep or remaining asleep. Hypersomnia – An individual experiences episodes of excessive daytime sleepiness or prolonged Narcolepsy – This is a chronic sleeping disorder and a chronic neurological disorder that is caused by the inability of an individual’s brain to regulate sleep-wake cycles properly. Restless leg syndrome – This is a neurological disorder where an individual experiences an unpleasant sensation in their leg (such as a creeping or tugging feeling). Sleep apnea – It occurs when an individual has one or more pauses in breathing while sleeping. Other sleep issues include mental disorders (depression or anxiety), or existing medical conditions such as Parkinson’s disease, Huntington’s disease, Encephalitis, or Thyroid disease. Not all sleep disorders are related to a medical condition or disorder. Individuals can also experience nightmares, sleepwalking, night terrors, and teeth grinding which are also considered sleep disorders. Emotional, environmental, or physical changes can also cause sleeping issues. The changing of a job, moving to a new home, financial problems, and death or illness of a family member can result in sleep disorders. Sleep disorders can be caused by medications such as antihistamines (used to treat allergies) or corticosteroids (used to treat cancer) that can affect the central nervous system and cause an individual to experience trouble sleeping. A common cause of sleep disorder is an individual having trouble sleeping due to consumption of caffeine or alcohol. There are also work related issues that can cause sleep disorders – in addition to the most common work related issue which is stress. An individual that travels a great deal can experience jetlag due to time changes. Also, an individual who works shift work can experience sleep disorders when switching from one shift to another. An individual’s sleeping arrangements can also contribute to a sleep disorder. Sleep apnea is a disorder defined as a pause of breathing while asleep. There are three types of sleep apnea. Obstructive Sleep Apnea (OSA) is the most common. This type of sleep apnea is caused by an obstruction which stops the flow of air to the nose and mouth. The second type is Central Sleep disorder (CSA). This type of sleep issue involves the region of the brain and nerves that regulate breathing do not function properly which causes breathing to be impaired. The third and final type of sleeping issue is Mixed Sleep issue and is rare. The type of sleep issue is a combination of Obstructive Sleeping issue and Central Sleep problem Each pause in the breathing while sleeping is called an apnea. It is believed the first documented reference to sleep issue was described in writing entitled “The Posthumous Papers of the Pickwick Club” by Charles Dickens in 1837. In 1965 a French doctor, Dr. Gastault and his colleges studied individuals with the sleep issue which is now known as sleep issue. In 1981, a group of Australian doctors documented treatment of sleep issue patients with what continuous positive airway pressure during sleep (CPAP), one type of successful treatment that is used by sleep issue patients today. In 1990 the non-profit organization, The American Sleeping disease Association (ASAA), was founded. The purpose of this organization is to increase the understanding of sleeping disease. The ASAA works with other non-profit organizations and societies of health care professionals to assist in reaching those individuals that are undiagnosed with sleeping disease. If undiagnosed, and left untreated, sleeping disease can be life-threatening and lead to an increased risk of heart disease and stroke. In 1988, the ASAA created the A.W.A.K.E. (Alert, Well, And Keeping Energetic) Network. This network plays a role of advocacy and education about sleeping disease. This network is made up of hundreds of groups in almost every U.S. state. The network provides advice about types of sleeping disease therapy, sleeping disease treatment, weight loss, and new research findings. ASAA publishes a newsletter entitled The Wake-Up Call which provides useful medical information about the disease. Sleep problem is not a rare disorder, but it is often an undiagnosed disorder. An individual with sleep problem is not aware they are not breathing properly. Even though individuals with sleep problem notice symptoms associated with sleep problem, most of the time individual’s with sleep problem seek medical attention because of symptoms noticed by their sleep partner. Sleep problem is diagnosed by a sleep study test called polysomnography (PSG) which is conducted by sleep specialist at a sleep clinic. The PSG test records the bio-physiological changes while the patient sleeps. The PSG monitors the brain waves, eye movement, muscle activity, and heart rhythm of the patient. A patient is said to have sleep apnea if the PSG test defines five or more episodes of apnea (pause in breathing) per hour. Sleep Apnea is a serious sleep disorder that can be life threatening and at times fatal. People with sleep apnea often fall asleep normally; however, once asleep their ability to breathe is blocked. Usually this inability to breathe is caused by the muscles in the throat relaxing too much and collapsing into the airway. The body then sends a signal to the brain that breathing has temporarily been blocked. This causes the person to wake up and start breathing again. The cycle of interrupted breathing can occur many times throughout the night. These episodes can occur up to 50 times an hour and last for ten seconds or longer. Often the person that suffers from this sleep disorder is unaware that anything is happening to them. They can not understand why they always feel tired during the daytime. The most common form of this sleeping disorder is obstructive sleep apnea, known as OSA. Some sufferers of obstructive sleep apnea also suffer from cental sleep apnea. This sleeping disorder is then called mixed sleep apnea. It is believed that obstructive sleep apnea affects between 18 – 20% of the adults in the United States. If this sleeping disorder is left untreated in can become life threatening and in rare cases fatal. It is the underlying cause of illnesses such as heart disease, stroke, pulmonary hypertension and systemic hypertension. There are several method of treatments for sleep apneas, both surgical and noninvasive. The first line of therapy for someone suffering from moderate to severe sleeping apnea is called positive airway pressure. Known as PAP, positive airway pressure is a noninvasive form of treatment. A machine delivers a constant flow of air through a mask that is worn while sleeping. The force of the air flow must be determined by a sleep technician during an overnight sleep study. There are three types of positive air pressure therapy, CPAP, BiPAP and AutoPAP. Dental devices are also used to treat this sleeping disorder in mild to moderate cases. Dental devices fall into two general categories: mandibular, or lower jaw, advancing devices or tongue retaining devices. Mandibular devices are used most often. They attach to the upper jaw and pull the lower jaw and base of the tongue forward. This shift in position keeps the airway open. Medications are generally not a successful form of treatment for most people with sleep apneas. However, many of them do take antidepressants and mondafinal. Supplemental oxygen is often used in conjunction with a PAP machine. Oxygen alone can not prevent the collapse of the airway or sleep fragmentation. However, oxygen can prevent the drop in the level of blood oxygen that occurs when the airway collapses. There are also surgical treatments for obstructive sleeps apnea that may be an option for some sufferers of this sleep disorder. These include uvulopalatophrayngoplasty, somnoplasty, corrective jaw surgery, palatal implants and tracheostomy.
